Please be aware that the preferred way of ordering medical certificates (sick notes) is now online. Please go to the practice website (search for - The Healthcare Centre, Preston) and on the Home page there is box which says, "Contact us On line" and click on "Submit a new request".

Please put the date due, how long for and the medical reason.

If you have no access to a computer, please continue to ring the surgery.
